State law requires that all children must have a health exam within 18 months prior to, or 90 days after entering first grade.
The health exam may be performed by your family physician or Managed Care provider, or by other CHDP Program providers. Your child may qualify for a CHDP school entrance exam at no cost if your family meets state income guidelines and a CHDP provider does the exam.
When you take your child for the exam, please take the state approved
Report of Health Examination for School Entry form (PM171A)
. It must be signed by the health examiner and returned to the school to verify that the health exam was completed.
A new law also requires that by May 31st of each year, students need to have a dental exam by a licensed dentist or other licensed or registered dental health professional in either kindergarten or first grade, whichever is his or her first year in public school. Children who have a dental exam within one year before entering school may meet this requirement. If you do not want your child to have a health or dental exam, you must sign a waiver to this effect. A waiver form may be picked up at your child’s school.
