Healthy Birth Outcomes[HBO]
 

A voluntary home visitation program using a multidisciplinary team of public health nurses, social worker, and community health workers who will provide intensive case management services. Program will focus on improving birth outcomes, infant health, and maternal/infant support systems by providing comprehensive assessment, development of a service plan in collaboration with client, referral to and assistance in accessing prenatal care, medical/preventative health services, social services, infant care/parenting education, and support services.

Target Population: Pregnant women or teens who are less than 28 weeks pregnant and who are experiencing a high risk pregnancy, or whose pregnancy outcome would be improved with coordination of care and services.

Open to any pregnant or parenting adult or teen in Stanislaus County.
Costs & Fees
Services are funded by State and federal grants and Medi-Cal. No client is denied service due to inability to pay.
