Improve health and development outcomes
for children.
Services provide:
- Comprehensive initial and regularly scheduled
assessment of physical, psychological, developmental,
nutrition and medical needs of infant or child.
- Comprehensive initial and on going assessment
of adults parenting skills, psychological and health
care needs.
- Development of a case plan with input from care
giver.
- Education for care giver about child care and
preventive health needs and unique care needs of
the substance exposed infant.
Target Population:
Substance exposed infants/children and their mother
or caregiver.
