Child Health and Disability Prevention (CHDP)

Child Health and Disability Prevention (CHDP) is a preventive health program that has providers who deliver periodic health assessments and services to low-income children and youth in California. These providers provide care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. Health assessments are provided by enrolled private physicians and community clinics.


What We Do?

As a local CHDP, the program works on informing, providing health education materials, provider recruitment, quality assurance, and client support services such as assistance with transportation and medical, dental, and vision appointment scheduling and encouraging the completion of an application for ongoing health care coverage. Local CHDP programs also oversee the Health Care Program for Children in Foster Care (HCPCFC).

To receive CHDP services, children and youth must live in California and meet one of the following requirements:

  • Up to 21 years of age and on Medi-Cal.
  • Uninsured children and youth up to 19 years of age and from a low-income family.
  • Children between the ages of 3 and 5 years and attend a Head Start or state preschool.
  • Children in foster care.


What We Offer?

CHDP provides resources for populations needing resources for CHDP medical, dental, vision providers. The program aims to increase prevention of health issues that can arise in childhood.
