Child Health and Disability Prevention (CHDP)

Child Health and Disability Prevention (CHDP) is a preventive health program that has providers who deliver periodic health assessments and services to low-income children and youth in California. These providers provide care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. Health assessments are provided by enrolled private physicians and community clinics.

Children with Medi-Cal Managed Care Plans or Fee for Service Medi-Cal

A Medi-Cal application will need to be completed to maintain coverage. Please speak with your child’s Medi-Cal healthcare provider to apply.
