Clinic Transition
Stanislaus County Health Services Agency will be making changes to some of the health care and public health services provided effective July 1st, 2024. The services and locations impacted are listed below:
Paradise Medical Office
The Health Services Agency will no longer operate the Paradise Medical Office at 401 Paradise Rd in Modesto. Golden Valley Health Centers (GVHC) will take over and continue offering health services at the same location with many of the same providers. They will continue the family medicine residency program and all patient care services currently offered. Patients currently assigned to the clinic by their managed care plans will automatically be reassigned to the new GVHC site.
McHenry Medical Office
High Risk Obstetric and Gynecological Services will no longer be performed at McHenry Medical Office, located at 1209 Woodrow Ave, Ste B-10, in Modesto. With many of the same providers, these services will continue to be available through Golden Valley Health Centers at their 1500 Florida Avenue, Modesto, CA Women’s Health location. Other services will continue as normal at the McHenry Medical Office.
HSA Specialty Clinic
The HSA Specialty (Orthopedic, Neurosurgery, Sports Medicine, and Surgery), Physical Therapy, and Wound Care Clinics located at 1524 McHenry Ave in Modesto will no longer be operated by the Stanislaus County Health Services Agency. Apart from Neurosurgery and Diabetes Specialist consults, which will no longer be available, Golden Valley Health Centers (GVHC) will take over and continue offering health services at the same location with many of the same providers. These services will continue to be open to outside referrals under GVHC.
Public Health Immunization Clinic
The Stanislaus County Public Health Immunization Clinic will move to 1524 McHenry Avenue, Suite 300. The clinic will operate Monday, Wednesday, and Friday from 10 am to 4 pm. Appointments are preferred, but limited walk-ins will be available. The clinic will be closed from 6/20/24-6/30/24 for relocation.
The Stanislaus County Health Services Agency will continue to operate two health centers: the McHenry Medical Office at 1209 Woodrow Ave and the Family and Pediatric Health Center at 830 Scenic Drive in Modesto.
Patient medical records will remain on file with the Stanislaus County Health Services Agency.
