Community Health Assessment
The Community Health Assessment (CHA) provides a comprehensive profile of the health and wellbeing of Stanislaus County. It highlights both the strengths and areas of improvement for the county and is a great resource for residents, partners, and key stakeholders to help them understand the health of the community. The CHA serves as the foundation for the actions outlined in the Community Health Improvement Plan.
The 2025 CHA was completed with help from many community partners, led by HSA/PH and the Stanislaus County Health Coalition Steering Committee. The assessment uses data and community feedback to help answer important questions: What health concerns are affecting Stanislaus County? Where are we doing well, and where do we need to improve? Community Health Improvement Plan
The Community Health Improvement Plan (CHIP) is a three-year, community-driven roadmap designed to address key health concerns identified in the Community Health Assessment. Developed through active collaboration with local organizations, stakeholders, and residents, the CHIP is a reflection of our shared commitment to building a healthier Stanislaus County, tailored to our community’s needs and challenges.
The 2025-2028 CHIP is centered around three strategic priority areas, chosen based on local data and community input:
- Access to Care – addressing barriers that prevent residents from obtaining timely, affordable, and culturally responsive health services.
- Behavioral Health – supporting prevention and mild to moderate behavioral health intervention.
- Chronic Disease – reducing the burden of chronic conditions through prevention and better disease management.
